Origins

Dungeons appear to be formed in various ways.

Natural Disasters

Natural Disasters (Japanese: 自然変動), such as blizzards, earthquakes, or volcanic eruptions, are known to be connected to the appearance of dungeons. Tiny Woods is a dungeon that appeared inside of a fissure. It is possible that the Unseen Force (or Gust of Wind) known to blow Pokémon out of dungeons is related to natural disasters.

According to Whiscash in Natural Disasters and Mystery Dungeons(UT) - This is an unofficial translation., natural disasters, and the subsequent appearance of dungeons, used to happen in the past as well.

Destruction of Time

The Destruction of Time causes the appearance of mystery dungeons.

(Explorers of Sky, before Spoink job) Chatot: "Time getting out of whack has caused an outbreak of bad Pokémon. Perhaps because of all that... We have noticed an increase in the number of jobs. In addition... and it's unknown if this is because of time's influence... There has also been a mass outbreak of ...mystery dungeons."

(Explorers of Sky - Beyond Time and Darkness) Narrator: "However, at some point, time began to show abnormalities, and because of this, strange places called Mystery Dungeons began to appear throughout the land. And then, deep within this world, some Pokémon became confused, and some Pokémon violent."

Dreams

Dungeons may form inside a Pokémon's dreams, as seen with The Nightmare. It is said by Darkrai that Distortions of Space lead to the appearance of such dungeons, although his claims are dubious, and the dungeon inside the dream was more likely made by him using his powers instead.

Mysteriosity

Areas with high Mysteriosity are known to have dungeons. Ley Lines affect and manipulate Mysteriosity in an area. Dungeon researchers can manipulate ley lines to create dungeons.

(Gates to Infinity, after buying the land) Partner: "The Mystery Dungeons are spreading around here... You never know what might happen on land like this."

(Gates to Infinity, day after the house is built) Partner: "And of course this area does have very high mysteriosity! Y'know? Like, it's really mysterious. If we start digging around, we might find that there are dungeons or things!"

(Gates to Infinity) Virizion: "Because no one has ever gone to the Great Glacier. Or that's... what they say... There are vast crevasses that make it impossible to reach the Great Glacier. And because the mysteriosity is so great there... ...the crevasses change unpredictably and no one can even map them. If you try to fly over the crevasses to get there, a freezing wind pushes you back."

(Gates to Infinity) Umbreon: "They're called Entercards. See, everyone knows that the land around here has unusually high mysteriosity... But even the mysterious power in these kinds of areas does obey some rules. It follows certain paths. What we call ley lines. The flow of these ley lines affects and determines the mysteriosity in an area. Once we realized that, dungeon researchers like Espeon and I began to wonder... What if we could use the power of those ley lines for ourselves?"
